A Giant Kidney, a Complex Surgery, and a Successful Outcome at Manipal Hospital Goa
Panaji: A 45-year-old man with Autosomal Dominant Polycystic Kidney Disease (ADPKD) (an inherited condition in which numerous fluid-filled cysts develop in the kidneys) was treated at Manipal Hospital Goa for a giant polycystic kidney measuring nearly 30 cm.
The patient had been experiencing severe pain in his side due to bleeding and rupture of a kidney cyst (when a fluid-filled sac in the kidney breaks and bleeds). Imaging scans showed that one kidney had become massively enlarged, occupying a significant portion of the abdomen. Multiple cysts were also seen involving the liver.
The size of the kidney left very little room to safely perform a minimally invasive procedure. Dr. Ankit Vyas, Consultant – Urosurgeon, therefore performed an open nephrectomy (surgical removal of the kidney through an incision in the abdomen).
The procedure was particularly challenging because the numerous cysts had significantly altered the normal anatomy around the kidney. The surgical team had to carefully identify and manage the surrounding blood vessels and tissues while removing the enlarged organ. Despite the complexity, the kidney was removed with minimal blood loss, and the patient recovered well following surgery.

The case demonstrates the challenges that can arise when kidney cysts grow extensively and highlights the importance of timely evaluation when symptoms such as persistent pain or complications from cysts develop.
